A wound thats left untreated and uncared for gets swollen, irritated and infected. So, Hughes is suggesting that a dream left unattended grows more irritable by the second, until it eventually runs.

Hughes then asks another rhetorical question:

Like a cloying sweet?
Hughes uses carefully picked out words in line 8 (syrupy sweet) to form an alliteration.
In line 7 (Crust and sugar over), Hughes could be implying that a dream could harden up, like sugar, if one has no hopes in achieving it, or if one just tries to forget about it. Also, he could be implying that if one tries to succeed in living their dreams, by and by a certain amount of time, itll be harder to do it.

The heavy load in line 10 is symbolizing the slant of ones dreams being carried on their shoulders, and never being able to do anything about it.
